The HTPA online membership directory is comprised solely
of input data received from this online application.
Whether
your's is an initial or a renewal application, your
listing in the online directory will be a mirror image of
the information you submit with this electronic form
regardless of any information that may have been posted
in the directory previously.
All
Memberships are subject to Annual Renewal
All
memberships are valid for the calendar year. Members must
submit a renewal application and pay annual dues in the
month of December for the following year.
Field
Labels featuring an asterisk require Data Input
in order to submit this application.
The
form field for "Fax" is deliberately optional
to accommodate members that do not havea fax service, or
do not want that number displayed. If you wish to
have your fax number displayed in your directory listing,
be sure to enter it into the form below.
Cost
of Dues
The
cost of an annual membership is $100
per applicant per year.
Payment
of Dues
Membership
applicants can pay annual membership dues
by following the online payment link that
appears on the confirmation page
after submitting a completed application form.
HTPA
memberships are valid through December
31st of each year.
